Going by the year most likely a 954-0485. If you still have the old belt use a tape measure and measure the length of the belt. If it is around 97 inches you know that it will fit....

I think this guide will be helpful. Let everything dry out for some time, if there still is water inside the phone, otherwise get some rubbing alcohol and gently clean the motherboard with it and some Q-tips....

The factory surface mount USB port utilizes a support bracket / bridge that is connected to the port as well as the board itself. Why did my “new” board not come with one installed? I have a few spares but did not want to install them if the supplied USB port is somehow more efficiently supported without it....
