It looks like you separated the digitizer from the LCD. On the iPhone 5, and most iPhones for that matter, the LCD and Digitizer are fused (glued) together and come off the phone in one piece. Go back through this guide to see how it should have been removed. You will need to order a new screen assembly to repair your phone, you can order one here. Given that you applied quite a bit of force to your phone, I am also concerned that you may have damaged the logic board. You should carefully inspect the logic board for signs of Pry Damage (you can google it) and damaged connectors .
