My Playstation 4 has this issue, where if I turn it on, it tries to read the Hard Drive to assure it’s okay, stops at %24 completion, and then the fan spins faster and faster and faster until the PS4 eventually shuts off. I took it apart, put it back together again, and it still does the same thing. Is the motherboard bad? What is going on here?

Your PS4 is overheating. There are various reasons it could be doing this…plugged vents or heat sink fins, bad thermal paste, faulty connections between APU and motherboard, etc. The only way to know is to check over each of these things carefully. Also, be sure that when you put the heat sink clamp back on that you installed it right side up…we see a lot of them where people install them upside down. Also, be sure that each memory chip has only one thermal pad on it…if you accidentally have 2 on one thermal pad that can make it overheat. I hope this helps!

It could also be that the HDD is faulty, if it gets to the same point each time then the system works harder to get the information which makes the fans run faster. Before changing thermal paste and such just put in an old working hdd into the bay and start the machine to see how it reacts.
